How It Works
At Divergent, our mission is YOU! We believe in a holistic, hands-on approach to physical therapy for your injuries, pain, or limitations. In order to do so, we are considered an out-of-network provider. We do this for 3 reasons:

1.) YOU! This structure allows us to focus on you and your specific goals so that we can provide you with what our experience and expertise tells us is going to help the most each visit, not what somebody sitting in a cubicle determines is appropriate for reimbursement.

2.) Us! It gives us the freedom to be creative and specific in our treatments and deliver personalized, one-on-one sessions targeted to your needs. No techs, no aides, no double-booking.

3.) Our Partnership. You commit to us and we commit to you. This shared accountability helps us achieve better outcomes faster.​​
In order for us to provide the highest quality, one-on-one care that we can, we are a cash-based practice and are considered out-of-network for self-claims. This allows us to put the focus on YOU and prioritize YOUR needs and goals. You will have faster access to care and more individualized treatments that you need to succeed. If you have a high deductible insurance plan, it may actually be cheaper to use a fee-for-service provider like Divergent Sports Medicine.
How is that? As a result of increasing deductibles and PT copays, many patients who have high copays or have not yet met their deductible may end up spending less money out of pocket for our treatments compared to an in-network provider.​ Furthermore, as a result of progressively lower and lower reimbursements for in-network practices, the therapists in these clinics are more likely to have to see 2 or more patients every hour and they often use assistants or aides in their treatment structures. This may lead to the use of more passive treatment techniques and modalities which the patient can often do on their own time at home or the gym.
At Divergent, we believe our hands-on approach and undivided attention for hour-long sessions lead to better outcomes and an improved overall rehabilitation experience. Upon request, invoices/superbills can be provided for you to send self-claims to your insurance company. Our rates vary depending on the type of service. Please call (847)220-7623 to inquire.

Please inquire with your insurance company about your out-of-network benefits and other requirements before treatment. Some insurance plans, like an HMO, still require a referral in order to receive PT treatment.

Physical Therapy Services
Initial Evaluation

Full 60-minute session with your Doctor of Physical Therapy when we will get to know who you are, discuss your current injury history, explore your goals and what you'd like to return to. We will perform a detailed biomechanical assessment of the body parts and structures that may be involved in your symptoms to discover the root cause. All initial evaluations will involve treatment, as time permits, in order to help you manage your pain and get you started on your path to recovery.
Discovery Visit/Call

FREE (local residents)
​
Come in or call for a 10-15 minute discussion about your symptoms so we can be sure we are the right ones to help you and to feel confident that Divergent Sports Medicine is right for you. We want to form a partnership in your rehab.
Follow-up Visit
​
All follow-up visits are 60-minutes, one-on-one with the same Doctor of Physical Therapy. We begin with a brief re-assessment of your symptoms and a check-in on your progress. Manual therapy, strengthening, movement correction, education, and more will be utilized to best meet your needs for the day and provide long term results.

Wellness/Recovery Services
Dry Needling (30 min)
​
Session will involve a brief discussion of your symptoms and education about risks and benefits of dry needling. Treatment can vary based on involved muscles and structures.
Maintenance/Prevention
​
Extend beyond your physical therapy plan of care with us and continue to build strength and stability to reduce your chances of injury in the future.
Tune-up/Recovery (30 min)
​
During this session, we will work on general aches, pains, and soreness from athletic competition, training programs, running/cycling events. We utilize manual therapy, stretching, cupping, and more.

Do you take my insurance?Divergent Sports Medicine chooses to be a fee-for-service provider as we feel it promotes our ability to provide the highest quality of care. We are considered an out-of-network provider, however, depending on your deductible and out-of-network health plan benefits, you may submit a self-claim and be eligible to receive reimbursement from the insurance company. Please contact your insurance company and inquire about your out-of-network benefits and any pre-authorizations needed. Divergent Sports Medicine can provide you with the necessary documentation required upon request. We cannot guarantee reimbursement.

Do I need a referral to see you?No! Illinois has "direct access" for physical therapy services. This means that in most cases, you do not need a physician referral prior to receiving PT treatment. If you will be seeking reimbursement, some insurance companies still do require a referral, so please check with your health plan provider.

What are my payment options?We accept cash, check, and Zelle as payment options. Divergent is a fee-for-service provider with payment due at the time of your appointments. At this time, we are not set up to take card payments.

What can I expect on the 1st visit? What should I wear?At your initial evaluation, we will start by talking about you and your story in order to discover what your goals are and where you want to be. We will perform a comprehensive assessment of your concern(s) to explore the root cause of the issue and create an individualized treatment program. Treatment will be provided with our remaining time and any questions or concerns you still may have will be attended to. Please wear comfortable clothing that allows you to move freely.

What if I have to cancel/reschedule?We understand that life gets in the way. If you need to cancel or re-schedule, we ask that you give us at least 24 hours-notice. Cancellation within 24 hours of your appointment will result in a reminder the first time and a full session charge after the 1st reminder.

What is involved in submitting a self-claim?Request your superbill/invoices from Divergent. Complete a claim form from your insurance provider. These forms can often be found on the company's website. Keep a copy of your claim for your own records. Review claim form to ensure everything is correct and send it in. Wait. If claim has not been processed in timeframe given by insurance company, give them a call. Many insurance companies offer an online portal or app to their patients that make filing a self-claim quick and easy.
